Our office is OPEN!

MASKS are MANDATORY for all patients and companions 

Masks must cover your mouth and nose completely and be attached with ear loops or tie-on straps. 

• We kindly ask you NOT to wear Bandannas, Neck Gaiters, or Masks with Respirators. 

• Masks must be worn AT ALL TIMES while in our office, as well as all areas inside the building.   

• If you must remove your mask, please do so OUTSIDE the building.

All patients are asked to complete a short Covid-19 Screening Form

You can complete this form in advance on the day of your visit prior to arrival by downloading and printing our Covid-19 Screening Form by clicking the PDF icon to the right:

TEMPERATURE CHECKS are MANDATORY for all patients and companions upon arrival

Staff temperatures are checked at the beginning and end of each shift

​

We ask all patients to WAIT IN YOUR CAR instead of our waiting rooms whenever possible

Please call us when you arrive at (716) 648-2770 (OPTION 0).  Once your room is ready, we will call or text you to come up to the office and have you taken to the room shortly thereafter

We ask that patients SOCIALLY DISTANCE within our waiting rooms or the lobby outside our suites when you are unable to wait in a car.

Please spread out and maintain distances of at least 6 feet apart!  

  

We ask that COMPANIONS WAIT IN THEIR CAR unless their presence is required. 

In these situations we ask that the patient bring no more than 1 companion.

  

We ask that you postpone and reschedule your appointment and NOT TO ENTER our facilities

if you have ANY Covid-19 symptoms, including:

FEVER, COUGH, SHORTNESS OF BREATH, SORE THROAT, LOSS OF TASTE/SMELL, NAUSEA, VOMITING, CHILLS, MUSCLE ACHES, HEADACHE, DIARRHEA, FATIGUE, OR OTHER FLU LIKE SYMPTOMS

We may ask you to complete your existing Appointment via Telehealth (either by videoconference or audio)
